USD 470 Sports Physicals

USD 470 sports physicals will take place May 5, 2008 at the Ark City Clinic, 510 W. Radio Lane. Parents should accompany their child to the clinic between 6 and 8p.m. No appointment is necessary and the cost is $20.00.

Students should pick up a physical packet at the high school or middle school office between April 21 and May 5. The form should be completed and signed by the parent before attending the physicals on May 5, 2008. Take the physical packet with you to the clinic; do not return it to the school office. Parents should plan to accompany students to the clinic.

A fee waiver form, for students that qualify for free lunches will be available in the high school office from Mrs. Trenary or at the middle school from Ms. Crain. The form should be completed and returned to the office by May 1, 2008. This form will be returned to the student and should be taken to the clinic at the time of the physical.
